PLEASE READ CAREFULLY 

The individual named on this registration form (“you” or the “attendee“) wishes to attend the 2026 Family Medicine Forum and its related meetings and events (hereinafter referred to collectively as the “event” or “FMF”), as hosted by the College of Family Physicians of Canada (“CFPC”) virtually and in-person in Toronto, Ontario from November 11-14, 2026. As lawful consideration for being permitted to attend the event and the intangible value that you will gain by doing so, you agree to all the terms and conditions set forth in this agreement (the “Agreement“). 

FMF 2026 Cancellation Policies: 
    • Cancel by August 15th for a full refund, less a $150 processing fee
    • Cancel by October 1st for a 50% refund, less a $150 processing fee
    • Cancel after October 1st – registration fees are fully non-refundable
    • Donations, and workshops and event ticket purchases are non-refundable 
    • The CFPC is not responsible for travel, hotel or any other expenses that attendees may incur in connection with a cancellation.

Changes in attendance format – from FMF In-Person to Live Stream and On-Demand:
    • If a change is requested by October 1st: The daily in-person registration fees paid less the cost of the live steaming and on-demand fees less a $150 processing fee will be refunded
    • If a change is requested after October 1st: Attendees will be switched from the in-person event to the live stream and on-demand event, however no refund can be applied. If the live stream and on-demand fees are more than the daily in-person fees paid, the attendee shall pay the difference in fees
    • Donations, workshops and event ticket purchases are non-refundable and cannot be transferred to cover the cost of live stream and on-demand events
    • The CFPC is not responsible for travel, hotel or any other expenses that attendees may incur in connection with any changes they make to their attendance format.

Last Minute Requests made after October 31st:  
    • To switch attendance format, from in-person to live streaming and on-demand, please email: [email protected]. We will make best efforts to provide access within 48 hours of our receipt of the request
    • Any requests, aside from change of attendance format, will be responded to within a few weeks

Response Delays: Between November 1-15, 2026 any requests for deferrals for compassionate reasons will be reviewed and responded to within 4-6 weeks after FMF concludes. As a non-profit event, we thank you for understanding that 100% of funds collected are utilized for delivery of the current event, hence refunds cannot be offered.

Automatic CPD Credit Entry: You give permission to the CFPC to upload your continuing professional development (CPD) credits to your Mainpro+ account approximately 4-6 weeks after FMF on-demand concludes. Tracking is based on scanned attendance from in-person sessions, and automatically tracked by your login for live streamed and on-demand attendance.

Health and Safety Guidelines: You are requested not to attend any in-person sessions, meetings, or events if you are feeling unwell. Attendees will be subject to the Province of ON and/or the City of Toronto’s public health guidelines and restrictions at the time of FMF. The CFPC is not responsible for any travel, hotel or other incidental costs or losses that are incurred as a result of restrictions that may be implemented over the FMF dates.

Transportation:  You recognize there are inherent risks, hazards, and dangers involved in travel and transportation in getting to and from FMF and between any events and venues offered as part of FMF. You hereby acknowledge, assume, and accept any and all risks and legal liabilities related to travel and transportation including, but not limited to, possible risks of injury, loss of property, damages, or death. This applies to all transportation, whether secured independently or provided or sponsored by the CFPC.

Exhibitor Scanning Permissions: If attending FMF in-person, you give permission to the CFPC to share your contact information with ONLY THOSE exhibitors that YOU ALLOW to scan your badge during the conference. Specifically, if you allow an exhibitor or sponsor to scan your badge at their booth, you understand and agree that your contact information including name, email, company name, phone numbers, and address (as provided by you in your registration) will be shared with that specific exhibitor or sponsor so that they may communicate with you regarding their products or services.

Access Control Scanning: If attending FMF in-person, your badge maybe be scanned at various checkpoints for access control. The purpose of the access control scanners is to confirm you have registered for the day you are attending or for the specific event you are looking to attend, when pre-registration is required, and to report general data. No personal data is collected or disclosed from these access control scanners.

Session Scanning: To track Mainpro+ credits, your badge will be scanned as you enter each in-person session. If, for any reason, your badge is not scanned upon entry, please visit the session scanning desk onsite at registration to have it scanned. Your credits will be deposited into your Mainpro+ account approximately 4-6 weeks after FMF on-demand concludes.

CFPC’s Privacy Policy: The personal information the CFPC collects about you through your registration and attendance at the FMF will be handled in accordance with the CFPCs Privacy Policy.

Personal Information Disclosure: In certain circumstances, we may need to share the personal information provided in your registration with third parties who support FMF in fulfilling related event services (e.g. with hotel partners, registration providers, etc.). Disclosure of your personal information in these circumstances will be made to perform specific tasks on our behalf and is restricted using contractual arrangements that prevent third parties from disclosing or using your information for any purpose other than those with which we specify. If applicable, your address (as provided in registration) may also be shared with supplier and distribution partners, for fulfillment of any requested product deliveries.  By completing your registration, you acknowledge and agree to the disclosure of your personal information as described above, for the purposes of facilitating your participation in FMF and the delivery of related event services.

Photography: Photographs, videos, and audio recordings from FMF may be produced and may include your recognizable image. By participating in FMF in-person you consent to being photographed, video-taped and audio-taped and you authorize the CFPC and its partners to use any of these recordings for its activities and events, as well as for its print and electronic promotional material, including on the internet, via the CFPC or FMF websites or on other social media websites. You grant the CFPC the right and permission to use any such photographs/video or audio clips in which you may be included, in whole or in part, for such purposes. You waive any right that you may have to inspect and/or approve any such photographs/video clips. You transfer to the CFPC any right you may have to such photographs/video clips and waive moral rights, if any; and, you release and discharge the CFPC from any liability that may arise from the use of such photographs/video clips by the CFPC. All photographic materials become the property of the CFPC and may be displayed, distributed, or used by the CFPC for any purpose. 

Recordings: You may not, without the prior written permission of the CFPC, copy, record, reproduce, photograph, or display any part of the in-person, live streamed or on demand event.

Assumption of Risk and Release:
I am aware and understand that by attending the event, there exists certain inherent risks, including risk of serious injury, disability, death, or property damage. I am also aware of the contagious nature of bacterial and viral diseases, including COVID-19 and the risk that I may be exposed to or contract COVID-19 or other infectious diseases by attending the event, which may result in serious illness, personal injury, disability, death, or property damage. I understand and acknowledge that these risks may result from or be compounded by the actions, omissions, or negligence of the CFPC employees or others, including negligent emergency response or rescue operations of the CFPC or its contractors. The CFPC cannot guarantee that I will not be injured or become infected with any disease due to my attendance at the event. I acknowledge that I am voluntarily participating in the event with knowledge of the dangers involved. I hereby agree to accept and assume all risks of injury, illness, disability, death, or property damage arising from my attending the event, whether caused by the negligence of the CFPC or otherwise.

I hereby expressly waive and release any and all claims which I have or may in future have, against the CFPC, and its officers, directors, employees, agents, representatives, affiliates, shareholders, successors, and assigns (collectively, “Releasees”), on account of injury, illness, disability, death, or property damage arising out of or attributable to my attendance at the event, whether arising out of the negligence of the CFPC or any other Releasee or otherwise, including without limitation, breach of contract, or breach of any statutory or other duty of care. I covenant not to make or bring any such claim against the CFPC or any other Releasee, and forever release and discharge the CFPC and all other Releasees from liability under such claims.
