All FMF 2010 Presenters must complete, sign and return this form to Anne Tucci at atucci@cfpc.ca or Fax (905) 629-0893 (be sure to fax both pages).

Guidelines for FMF Presentations
Brief comments to assist you in preparing your presentation.
(Presentation content, handouts, evaluation, registration, timing, audiovisual requirements and room set.)

Media Information
Important information for FMF speakers in regards to the Media.

AUDIO VISUAL
A standard conference audiovisual equipment package will be supplied.
This includes a computer, LCD projector, screen and flipchart.
No a/v or room set changes or additions will be accepted after August 15, 2010.

HANDOUTS
Handouts summarizing key points and practice tips related to your topic are greatly appreciated by our registrants. With the presenter’s permission (complete, sign and return the Presenter Information Form), session handouts at Family Medicine Forum are provided to registrants in electronic format and available on the CFPC website after FMF.

All handouts must be provided (in pdf format) to Anne Tucci at atucci@cfpc.ca no later than Sept. 1st in order to be included in registrant kits.

GUIDELINES FOR SPEAKER HANDOUTS:
• Copyrighted material should not be included
• Recognizable images of people’s faces must not be included
• The number of pages should be kept to a minimum (not exceeding 10 pages)
• Include basic information only – key points and other data
• Provide a bibliography or reference list
• If using acronyms or abbreviations, please provide definitions
• No advertisements or other endorsements or sales items should be included
• Please provide in pdf format
