Proof of Participation - FMF 2009
A letter verifying attendance at Family Medicine Forum 2009 will be sent to each FMF registrant who had their name badge scanned by CFPC staff while at the conference. The letter will serve as your proof of participation.
Mainpro-M1 Credit reporting
All FMF attendees who submitted their 2009 FMF Evaluation and Mainpro-M1 Credit Reporting (either on-site or by fax or e-mail) can expect to see the credits claimed reflected on their record prior to the end of December 2009. If you have any questions about your CME record, please contact our Continuing Professional Development (CPD) department at md@cfpc.ca. If you need a copy of the Mainpro-M1 Evaluation and reporting form, please contact Michelle Gutkin at mg@cfpc.ca or 905 629 0900 x800.
Note: The form is for Mainpro-M1 credits earned during FMF program hours only. Certificates of participation/completion for Mainpro-C and satellite symposia sessions will be issued by the CME/CPD provider. To claim these credits, please use a CME Credit Reporting Form or report online (Go to www.cfpc.ca and click on “members” to access the online CPD reporting area).
